Athletic Forms

Athletic Code – This form must be signed by parent and athlete.  It outlines conference age requirements, grade eligibility, and game day uniform. Must be completed each year.

Athlete’s Pledge Form – Athlete honor code

Parent Pledge Form – Parent honor code

Athletic Transportation Form – Permission for athlete to ride with someone other than legal guardian.  Must be completed each year.

Athletic Participation Fee Form – Per sport athletic fee to cover officials, game day expenses, equipment, etc.

Athletic Uniform Policy – Outlines expectations regarding condition of uniforms.

Athletic Medical Consent Form – A doctor provided athletic form is also permissible.  Good for one year from date of physical.  Must bring a copy with you to tryouts.
